Childbirth

“The Childbirth and Childcare Lump-Sum Grant“ is paid when an insured person gives birth and “the Dependents' Childbirth and Childcare Lump-Sum Grant“ is paid when a dependents gives birth.

(Dependents') Childbirth and Childcare Lump-Sum Grant

The Childbirth and Childcare Lump-Sum Grant / Additional Sum will be paid when the childbirth in which the insured or dependents giving birth after 4 months or more (85 days) of pregnancy.
(For each child regardless of normal birth, stillbirth, miscarriage, premature birth)

Amounts paid 500,000 yen
  • *420,000 yen for childbirth through the end of March 2023
  • *Paid per child for multiple child
  • *For childbirth (including stillbirth) after at least 22 weeks of pregnancy under the medical administration of a medical care institution or other institution that is a member of the maternity medical care compensation scheme; 488,000 yen (408,000 yen for childbirth through the end of March 2023) for childbirth at an institution that is not a member of the maternity medical care compensation scheme

The maternity medical care compensation scheme

This system pays compensation when a newborn child develops cerebral palsy despite being born through normal pregnancy and birth. The Japan Council for Quality Health Care manages the system, and most medical institutions providing childbirth services are members.
Childbirths eligible for compensation must meet all of conditions (1)-(3):
(1) childbirth following at least 28 weeks of pregnancy;
(2) case of cerebral palsy equivalent to a grade 1 or grade 2 physical disabillity;
(3) case of cerebral palsy not due to congenital factors of factors arising in the neonatal period (Evaluation criteria for compensation differ depending on the date of birth.See this reference link below).

System of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions

The system in which a medical institution applies for and receives a lump-sum payment for childbirth and childcare on behalf of the insured is called the "direct payment system".
The health insurance association pays the lump-sum birth allowance to the medical institution based on the request from the medical institution, so there is no need to apply for the lump-sum birth allowance to the health insurance association.
If the childbirth cost is less than the lump-sum childbirth and childcare allowance, the difference and additional benafits will be paid to the insured through the company's salary, so there is no need to apply to the health insurance association.
When you giving birth at a medical institution that can use the direct payment system, the medical institution will explain the direct payment system by the time you leave the hospital, and you can use the direct payment system if you wish.

System of receipt on Your Behalf

The direct payment system may not be introduced in small medical institutions.
Even such medical institutions may have a ”receipt agency system” as an alternative system that can reduce the amount of payment at the counter.
The receiving agency system is a system in which the insured and the medical institution apply for the use of the system to the health insurance association in advance and then the medical institution receives the lump-sum payment for childbirth and childcare on behalf of the insured.
If the cost of childbirth is less than the lump-sum birth allowance + surcharge, the insured person will be automatically paid later through the company's salary, so no procedure is required.
