Procedures:	Registration is required through the company's system. Please contact the Human Resources Division of Microsoft Japan “askjphr@microsoft.com” for information on how to register in the system. After registering in the system, you will receive a document kit at your home. Please follow the instructions in the kit and submit the required documents.
Deadline:	Immediately after the name change
Submit documents to:	The address on the return envelope in the return kit. If you have not received the kit more than one month after you submitted your application, please contact Microsoft HR about it. (askjphr@microsoft.com)

**[In the case of :Voluntarily and Continuously Insured Person]**
Procedures:	Please submit the required documents.
Required documents:	Notification of Change of Registration Matters for a Voluntarily and Continuously Insured Person
Deadline:	Immediately after the name change
Submit documents to:	Microsoft Health Insurance Association, Shinagawa Grand Central Tower, 2-16-3 Konan, Minato-ku, Tokyo 108-0075
